چکیده

▼ Introduction: The eff ects of escitalopram (10 mg/d) coadministration on plasma concentrations of aripiprazole and its active metabolite, dehydroaripiprazole, were studied in 13 Japanese psychiatric patients and compared with those of paroxetine (10 mg/d) coadministration. Methods: The patients had received 6–24 mg/d of aripiprazole for at least 2 weeks. Patients were randomly allocated to one of 2 treatment sequences: paroxetine-escitalopram (n = 6) or escitalopram-paroxetine (n = 7). Each sequence consisted of two 2-week phases. Plasma concentrations of aripiprazole and dehydroaripiprazole were measured using liquid chromatography with mass spectrometric detection. Results: Plasma concentrations of aripiprazole and the sum of aripiprazole and dehydroaripiprazole during paroxetine coadministration were 1.7-fold (95 % confi dence intervals [CI], 1.3–2.1, p < 0.001) and 1.5-fold (95 % CI 1.2–1.9, p < 0.01) higher than those values before the coadministration. These values were not infl uenced by escitalopram coadministration (1.3-fold, 95 % CI 1.1–1.5 and 1.3-fold, 95 % CI 1.0–1.5). Plasma dehydroaripiprazole concentrations remained constant during the study. Conclusion: The present study suggests that low doses of escitalopram can be safely coadministered with aripiprazole, at least from a pharmacokinetic point of view. The quantitative detection of aripiprazole and its main metabolite by using capillary-electrophoresis.

BACKGROUND We aimed to establish a feasible and reliable method to measure the level of aripiprazole and its main metabolite, dehydroaripiprazole, using a capillary-electrophoresis (CE) machine. METHODS Two blood samples were obtained from psychiatric patients hospitalized in Yu-Li Hospital who had been treated with aripiprazole for more than 4 weeks, at least 10 mg/d. Conditions for voltage,...

Pharmacokinetics of aripiprazole, a new antipsychotic, following oral dosing in healthy adult Japanese volunteers: influence of CYP2D6 polymorphism.

We investigated the pharmacokinetics (PK) of aripiprazole, a newly developed antipsychotic, and its active metabolite in healthy Japanese, and the influence of CYP2D6 polymorphism on the PK of aripiprazole. Following a single oral 6 mg dose, the mean C(max), t(max), and t(1/2, z) (terminal phase half life) of aripiprazole were 31.0 ng/mL, 3.6 hr, and 61.0 hr, respectively. The t(1/2, z) in CYP2...

Comparison of the bioactivation potential of the antidepressant and hepatotoxin nefazodone with aripiprazole, a structural analog and marketed drug.

In vitro metabolism/bioactivation of structurally related central nervous system agents nefazodone (hepatotoxin) and aripiprazole (nonhepatotoxin) were undertaken in human liver microsomes in an attempt to understand the differences in toxicological profile. NADPH-supplemented microsomal incubations of nefazodone and glutathione generated conjugates derived from addition of thiol to quinonoid i...

Excretion of Acebutolol and its Major Metabolite Diacetolol into Infant Blood Circulation and the Breast Milk

Acebutolol (AC) is a chiral b-adrenergic blocking drug, which is useful clinically as the racemate in treating hypertension and is metabolized to an equipotent chiral metabolite, diacetolol (DC). In this paper we report a case of a 32 year old woman who was receiving AC during her pregnancy and lactating time for management of hypertension. The maternal plasma level and breast milk as well as c...
